Example: Train Crash

A train rolls over at high speed killing its driver and a high proportion of its occupants. The government calls an independent inquiry.
Professor Dix is asked by the CEO/Board to determine (urgently) if:
-there is an inherent safety issue with the train system?
-what (if any) urgent action should be taken?
-what was the cause?

Approach:

Professor Dix immediately deploys to crash scene. He investigates the crash site, the vehicle and all documentation he can find on pertinent issues. He travels by night and day with other train drivers investigating the culture and knowledge of other employees. He initiates engineering reviews and examines other rolling stock for clues.

Results:

Within 7 days of investigation advice to Board/CEO on likely cause of crash coupled with suggested modifications to rolling stock.
